What is IMPLICIT ICC?
IMPLICIT Interconception Care (ICC) is a brief, innovative model that screens women/birthing people at Well Child Visits from 0-24 months to address 4 modifiable risk factors. 

What is a modifiable risk?
Modifiable risks are behaviors that can be changed to lower one’s risk for poor health outcomes.

What does IMPLICIT ICC screen for?
• Smoking/vaping status
• Depression
• Family planning
• Multivitamin with folic acid use

What are IMPLICIT ICC interventions?
When a woman/birthing person screens positive for a risk factor, providers can choose from multiple intervention options for each risk factor. A full description of each intervention can be found in the IMPLICIT ICC toolkit. 

How many sites have implemented IMPLICIT ICC?
43 sites have implemented IMPLICIT ICC at their practice. Since the creation of the ICC model, IMPLICIT ICC has served 49,980 dyads, or women/birthing people and their babies.

What is patient centered contraception counseling?
Patient centered contraception counseling is a form of shared decision making between a provider and their patient regarding contraception. The model positions the patient as an expert on their body and the provider as an expert in medicine, thus giving the patient more autonomy in discussions about their care.

What is the 4th Trimester?
The 4th trimester is known as the first 12 weeks (or 3 months) after delivery.

What is the IMPLICIT 4th Trimester (4TM) Care Model?
IMPLICIT 4th Trimester (4TM) is a model of postpartum care that addresses the current needs for women/birthing people and protects against morbidity and mortality after pregnancy. 

What is an early postpartum visit?
An early postpartum visit is scheduled within 21 days (or 3 weeks) after delivery; it includes screening to assess for health risks. This visit is “early” because it is scheduled before the standard 6-8 week comprehensive postpartum visit. 

What is a comprehensive postpartum visit?
A comprehensive postpartum visit is scheduled within 6-8 weeks of delivery; it includes a comprehensive recovery assessment to see how a woman/birthing person is recovering after giving birth.

How many sites have implemented IMPLICIT 4TM?
4TM is a newer addition to IMPLICIT’s care models, currently 12 IMPLICIT Network sites have implemented 4TM at their practice and more are in the process of implementing.

What is the IMPLICIT Network?
The IMPLICIT Network is a project of the Family Medicine Education Consortium, a nonprofit membership association (FEIN 31-1436038).

A primary care maternal child health learning collaborative focused on improving birth outcomes and promoting the health of women, birthing people, infants, and families through innovative models of care, quality improvement, and professional development of current and future physicians.

  • Develops, implements, evaluates, and optimizes new and existing models of care focused on improving birth outcomes and the health of women/birthing people, infants and families
  • Supports implementation of ICC and 4TM models at clinical sites
  • Creates a professional forum for primary care providers that grounds conversations about maternal and infant health outcomes in evidence, equity and justice. 

What is an IMPLICIT Network Site?
An IMPLICIT Network site is a practice that has implemented IMPLICIT ICC or 4TM (or both!) with continued support from the IMPLICIT Network. Sites are classified in the following categories:

  • Active, sharing data: IMPLICIT Network dues paying site that has infrastructure set up to share data with IMPLICIT Network staff
  • Active, not sharing data: IMPLICIT Network dues paying site that does not currently share data with IMPLICIT Network staff
  • Emeritus: Honorary IMPLICIT Network site, not currently active

Who can become an IMPLICIT Network Member?
Any practice providing care to infants 0-24 months and/or postpartum women/birthing people is welcome to join the IMPLICIT Network! You do not have to be an academic residency site to join the Network. We welcome all who are interested in improving the health of women/birthing people at our organization.

  • Currently, our active Network consists of 28 academic residency sites, 14 non residency sites, and 2 pediatric sites.

We also offer affiliate memberships to individuals whose sites are not IMPLICIT Network but may be interested in utilizing IMPLICIT Network resources.

How do I join the IMPLICIT Network?
Sites interested in joining the IMPLICIT Network may reach out to our staff at implicitinfo@fmec.net to schedule an informational meeting. IMPLICIT Network staff supports implementation of ICC and/or 4TM care models at all new sites and offers continued support post implementation. What is quality improvement (QI)?
QI, or Quality Improvement, is “a continuous and ongoing effort to achieve measurable improvements in the efficiency, effectiveness, performance, accountability, outcomes, and other indicators of quality in services or processes which achieve equity and improve the health of the community” (CDC)

Meet our Leadership Council - today we're highligh Meet our Leadership Council - today we're highlighting Dr. Narges Farahi!

Dr. Narges Farahi is a family medicine physician and longtime champion for person-centered and family-centered care. Dr. Farahi is a Professor at UNC, where she served as the Director of the Family Medicine Perinatal and Child Health Program for ten years. Her heart is in community health, and her outpatient practice is at Prospect Hill Community Health Center, a rural federally qualified health center. Prior to her current role, Dr. Farahi served as the medical director of the NC Farmworker Health Program. Her clinical interests include providing seamless care for people with the capacity for pregnancy across the continuum of care, including before, between and after pregnancies. Dr. Farahi is committed to providing trauma-informed care, supporting people and families to meet their reproductive goals, and using quality improvement strategies to help health care practices and systems better meet the needs of patients and families. She has been a leader in implementing enhanced access to comprehensive contraception options and the IMPLICIT interconception care model in the primary care setting. Dr. Farahi works to integrate community engagement and implementation science to transform care with a focus on patients with hypertensive disorders of pregnancy and polycystic ovary syndrome. Her goal is to center the voices of patients and families from marginalized communities and build partnerships with patients and communities to improve health.

Juggling the responsibilities of life and parentho Juggling the responsibilities of life and parenthood often means less time to take care of yourself. Many mothers/birthing people (MBP) don't attend their own doctor's appointments, but almost ALL attend their child's doctor's appointments. 

Smoking/vaping status is one of the screening questions asked during both IMPLICIT Interconception Care (ICC) and 4th Trimester (4TM) models of care. Many times, MBPs who smoke or vape, see quitting during pregnancy as a time limited response and will start again after having the baby. 

IMPLICIT ICC screens mothers/birthing people at their child's Well Child Visits (WCV) from ages 0-24 months and offers the opportunity to meet MBPs where they are at, providing support in maintaining or starting their smoking/vaping cessation. 

IMPLICIT data showed that an overwhelming number of MBPs are willing to receive advice about their health from their child's provider. 

Identifying risks and providing interventions at WCVs in between pregnancies promotes not only healthier future pregnancies, but a healthier mother/birth person, child, and family.
