I saw a patient during her prenatal care as well as for 4th trimester visits. Her contraception plan was to have a bilateral tubal ligation (BTL), we identified this during the prenatal period, and were able to have her scheduled to get her BTL about 6 weeks postpartum. We discussed options for bridging contraception and the patient preferred to use condoms. She received her BTL as planned and found out 3-4 weeks after her BTL that she was pregnant. This news was a big shock for her and she wanted to consider all of the available options. She was in denial about the pregnancy for a long time and chose not to seek prenatal care, but she brought her daughter regularly to her well child visits (WCVs). During the ICC portion of the WCVs, I was able to discuss options for the pregnancy with mom, order her initial prenatal labs and ultrasound, and refer her to counseling. While she ultimately did not come into the office for a prenatal visit until after 20 weeks of pregnancy, I saw her several times prior to that visit during her daughter’s WCVs and was able to have an ongoing discussion with her about options as well as provide care for her immediate needs. The relationship that the patient and I had formed during her prenatal care was important for helping us to continue to work together during her daughter’s WCV and ultimately led to her feeling comfortable coming to establish prenatal care again for her unexpected pregnancy.
